LICENSE TRANSFER FROM DHA TO HAAD FOR ALL HEALTH CARE PROFESSIONALS:

Hai , its a good news about license transfer from one autghority to another authority with in UAE. Hereby I am mentioning about procedure for transfering license from DHA  to HAAD for health care professionals.

 DOCUMENTS REQUIRED FOR LICENSE TRANSFER:

1. Passport copy
2. photograph with white back ground
3. Degree /Diploma certiifcate
4. Transcript or mark list
5. High school or Higher secobndary certifcate
6. Home country license (Initial copy and renewed )
7. DHA  License card
8. Experience certificates from DHA  facilities (not less than 6 month)
9. Other experience certificates
10. NOC from current facility if you are still working
11. Good standing certiifcate from DHA
12. Basic Life support(optional for soem professionals)
13. Professional CV
14. Old dataflwo Report
15. Letter of authorization
16. Facility declaration form
17. Declaration form by applicant
18. Staff data form
19. CID electronic barcode
20. Offer letter from any of the facility
21. Log book for surgeons etc

PROCEDURE:

• Sign up for HAAD Data flow
• If you have facility already under haad , then select facility name while doing dataflow  for amking it easier. If you dont have any facility tehn start as normal dataflwo procedure
• Do old dataflwo transfer and add additioanl document for veriifcation(DHA goodstanding, DHA experience and DHA license)
• After dataflwo completion you will get HAAD login credentials.Sign up and attach all documents
• Once you find out a job, your facility will open your application and will proceed for license transfer .

note: Those who attended HAAD exam and failed, can't transfer . You have to prove that you possess the  capability to pass assessment.
